Appointments & billing
To see a provider in our clinic, use MyChart to schedule online anytime, or call any of our locations during office hours. Please arrive at least 15 minutes before your appointment. Bring:
- Your photo ID
- Your insurance card
- Your method of copayment or payment in full if insurance will not cover your visit
- A list of any medications you are currently taking, including name and dosage
- Any forms that you need our office to complete
- Any forms you have completed ahead of time using the links below
To collect necessary information before your visit, we ask that you complete the following forms. If you do not complete them ahead of time, they will be available at your visit.
We suggest you confirm coverage with your insurer before your visit. Below is a list of insurance plans commonly accepted by Novant Health clinics.
